Under general supervision, provides case management to older adults who reside in Baltimore County.
Essential Job Duties
Graduation from an accredited college or university with a bachelor's degree in a social or behavioral science.
Experience as described in essential duties may be substituted on a year-for-year basis up to a maximum of four years for the required education.

Proof of Licenses, Certifications and Education
Applicants are required to submit proof of education to meet the required and preferred qualifications of the position. Diplomas or transcripts must show the applicant's major field of study. Copies and unofficial transcripts are acceptable. Proof of professional license(s) certifications, and/or education must be submitted with each application.
Licenses and Certificates
Some positions within this classification may require possession of a valid driver's license equivalent to a non-commercial, class C Maryland driver's license at the time of appointment.
Some positions within this classification may require satisfactory completion of County sponsored CPR and First Aid training.
Licenses and certifications must be maintained throughout employment.

EXAMINATION PROCEDURE
Qualified applicants will be assigned a rating based on an evaluation of their training and experience as submitted on their application and supplemental questionnaire. The duties of this position are essentially sedentary with occasional walking, lifting, or other restricted physical activities. Duties will require traveling between work sites.

This position requires satisfactory completion of County sponsored CPR and First Aid training.
Possession of a valid driver’s license is required at time of appointment.
Licenses and certifications must be maintained throughout employment.
Medical Examination and Employment Background Investigation
Applicant's selected for an appointment to a position in Baltimore County must successfully complete a physical examination, drug screen and an employment background investigation, including, but not limited to a criminal background, education and fingerprint check.

- Performs client related field visits/ assessments and completes paperwork required for those assessments.
- Transports and accompanies clients on medical appointments.
- Shops for clothing or personal needs for clients.
- Maintains client records in electronic format.
- Completes administrative tasks to include, compiling statistical data, completing tasks related to p-card purchases and mileage.
- Attends meetings and training sessions.
- Maintains expertise in the field of aging, resources for older adults, and public benefits.
- Performs other related duties as required.

- Ability to make appropriate referrals.
- Ability to establish and maintain good working relationships with others.
- Ability to appropriately handle hostile individuals and those experiencing a crisis.
- Ability to enter and maintain client information in electronic records.
- Ability to make public presentations
- Ability to interpret Federal, State, and local regulations, procedures, and guidelines for human services professionals.
- Ability to read and understand professional journals and other publications.
