As a Patient Access Coordinator, you will be responsible for the overall patient access process, which includes answering calls, responding to incoming referrals in an efficient manner, and coordinating assessments. You will also collect necessary documentation and information, such as consents, insurance details, and government-issued IDs, prior to assessments. Clear communication is essential as you will coordinate with both the outreach and clinical assessor teams to ensure smooth hand-offs and thorough documentation. Your role will ensure the effective flow of patient care and prioritize access to treatment while maintaining confidentiality and following company guidelines.
PRINCIPAL DUTIES AND JOB RESPONSIBLITIES
- Answering Incoming Calls: Respond to calls from patients, families, and referral sources, providing information about available services and intake procedures.
- Handling Referrals: Respond to all incoming referrals in an efficient manner, gathering the necessary information to determine patient needs and ensuring referrals are processed promptly.
- Coordinating Patient Assessments: Work with clinical assessors to schedule patient assessments, ensuring all necessary information is gathered and provided before the assessment.
- Obtaining Necessary Documentation: Collect and verify patient information, including consents, insurance details, and government-issued IDs, ensuring all required documents are completed before assessments.
- Providing Outbound Referrals: Coordinate and initiate outbound referrals when necessary, ensuring that patients are directed to the appropriate services or healthcare providers.
- Documenting and Tracking Patient Access: Accurately document patient access information in the system, adhering to confidentiality guidelines and company protocols.
- Follow-Up Coordination: Schedule follow-up appointments or calls as needed, ensuring patients and referral sources are contacted in a timely manner.
- Thorough Communication with Outreach and Clinical Assessor Teams: Maintain clear communication with outreach and clinical assessor teams, ensuring smooth hand-offs and all required information is accurately communicated.
- Ensuring Clear Documentation and Handoffs: Document all interactions thoroughly to ensure clear communication between all teams, including notes about consents, insurance, and patient details for clinical assessment.
- Responding to Patient and Referral Questions: Answer patient and referral source questions regarding service availability, referral processes, and other related inquiries. Strong knowledge and ability to communicate the mental health levels of care:
- Inpatient Hospital
- Residential Treatment Center
- Partial Hospitalization Program
- Intensive Outpatient Program
- Outpatient Program
- Maintaining Records: Ensure that all records, notes, and documentation are maintained accurately and securely, in compliance with organizational policies.
- Adherence to Policies and Procedures: Follow all company guidelines, including privacy policies, safety protocols, and any other relevant procedures.
· Other Assigned Duties: Perform other related duties as assigned by the Director of Patient Access.

7AM-7PM, Rotating shift, 4 on 3 off, 10-hour shifts
