Job Description:

• We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a Hospice Care Provider in New Jersey.
• The ideal candidate will have experience working with patients requiring palliative care, with a strong focus on providing emotional support and comfort during challenging times.
• The successful applicant will be responsible for assessing patient needs, developing personalized care plans, and collaborating with interdisciplinary teams to ensure comprehensive care delivery.

Responsibilities:

• Conduct home visits to assess patient conditions and develop targeted care plans
• Administer medications, treatments, and procedures as ordered by physicians
• Collaborate with healthcare teams to coordinate patient care and address complex needs
• Provide emotional support and education to patients and their families

Requirements:

• Current RN licensure in NJ
• Minimum one year of recent clinical experience, preferably in hospice or home health care
• Ability to work independently, manage time effectively, and prioritize multiple tasks
• Strong interpersonal skills and effective communication abilities
