• Provide skilled physical therapy services to clients in their homes, including exercises, stretches, and other interventions as prescribed by the supervising physical therapist.
• Conduct thorough assessments of clients' physical abilities and create detailed reports to inform care planning decisions.
• Collaborate with other members of the healthcare team to ensure seamless coordination of care and optimal outcomes for clients.
• Maintain accurate and up-to-date records of client interactions, including progress notes and treatment plans.
