The successful candidate will be responsible for conducting home visits, developing and implementing individualized care plans, educating patients and family members on rehabilitative care, and accurately documenting observations and interventions. They will also collaborate with other healthcare professionals to ensure comprehensive patient care.
Qualifications for this role include a current Maryland Physical Therapist license, graduation from a program approved by the American Physical Therapy Association, and strong interpersonal and organizational skills. Prior experience with electronic medical records (EMR) is preferred.
