BAYADA Home Health Care is seeking a Physical Therapist for a Full Time opportunity performing home health visits for our clients in Northern Anne Arundel County.
The successful candidate will be an integral member of a multi-disciplinary healthcare team providing rehabilitative care and skilled nursing to clients, affording them the opportunity to receive medical care required to remain at home.
Responsibilities
- Make home visits to clients in designated geographic territories.
- Perform diagnostic tests and measurements.
- Develop and implement individualized care plans.
- Continually assess and revise the care plan.
- Educate and instruct clients and their representatives on rehabilitative care.
- Accurately document observations, interventions, and evaluations.
Qualifications
- A current Maryland Physical Therapist license.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills; prior experience with electronic medical records preferred.
