Job Title: Home Health Physical Therapist
Job Summary
This is a full-time opportunity for a skilled and compassionate Physical Therapist to join our multidisciplinary healthcare team in providing high-quality rehabilitative care to adult and geriatric clients in the comfort of their own homes.
Responsibilities
- Conduct thorough assessments of clients' physical abilities, identifying areas of strength and weakness, and develop personalized treatment plans to promote optimal recovery and independence.
- Implement evidence-based interventions, including manual therapeutic exercises, gait training, balance, and other activities, to address specific client needs and goals.
- Educate and instruct clients, family members, or caregivers on safe and effective strategies for managing daily activities, preventing complications, and promoting overall well-being.
- Collaborate with nursing staff and other healthcare professionals to coordinate care and ensure seamless transitions between services.
- Accurately document all observations, interventions, and evaluations in electronic medical records, utilizing state-of-the-art technology to enhance efficiency and quality.
Requirements
- Current licensure as a Physical Therapist (PT) in Pennsylvania.
- Graduation from an accredited physical therapy program approved by the American Physical Therapy Association.
- Ability to work independently, manage time effectively, and prioritize tasks to meet deadlines.
- Strong interpersonal, organizational, and problem-solving skills, with excellent communication and documentation skills.
- Experience with electronic medical records (EMR) and touch pad tablets preferred.
