Job Description
The ideal candidate will possess a strong background in speech-language pathology, with excellent communication and interpersonal skills. As a home care Speech Language Pathologist, SLP, you will work closely with clients, families, and healthcare professionals to provide high-quality rehabilitative care and support.
Key Responsibilities:
- Conduct thorough evaluations to assess, diagnose, and treat clients with speech, language, communication, or swallowing difficulties.
- Develop and implement individualized treatment plans, collaborating with healthcare teams as needed.
- Educate patients, family members, and caregivers on rehabilitative care and activities necessary for optimal health and independent living.
- Accurately document observations, interventions, and evaluations using electronic medical records (EMR) systems.
Qualifications:
- Current license as a Speech Language Pathologist in PA.
- Certificate of clinical competence in speech from the American Speech and Hearing Association or equivalent educational requirements.
- Solid computer skills and experience with electronic medical records (EMR) preferred.
