About the Role:
- Make home visits to clients in designated geographic territories.
- Perform diagnostic tests and measurements to assess client needs.
- Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
- Educate and instruct clients, family members, or caregivers on rehabilitative care and activities necessary for independent living.
To be successful in this role, you will have a current New Hampshire Physical Therapist license, a minimum of one year of recent work experience as a Physical Therapist, and strong interpersonal, organizational, and problem-solving skills. You will also be able to work independently and manage time effectively.
