At BAYADA Home Health Care, we are seeking a skilled Home Health Therapist to join our team. As a Physical Rehabilitation Specialist, you will be responsible for making home visits to clients in designated geographic territories.
Job Responsibilities:
- Conduct diagnostic tests and measurements to assess clients' mobility, strength, and balance.
- Develop and implement individualized care plans, including manual therapeutic exercises and gait training.
- Continually assess and revise the physical therapy care plan, participating with nursing in the multidisciplinary care plan as necessary.
- Educate and instruct clients, family members, or representatives on rehabilitative care and activities promoting health, safety, and independent living.
- Accurately document observations, interventions, and evaluations related to client care management and services provided.
Qualifications for this role include:
- A current New Jersey Physical Therapist license.
- A minimum of one year of recent work experience as a Physical Therapist.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills; prior experience with electronic medical records preferred.
