Per Diem Registered Nurse (RN) – Home Health Visits
We are seeking a compassionate RN to provide skilled nursing visits, assessments, and admissions to adult and geriatric patients in a home setting.
About the Role
- Provide nursing care to adult and geriatric patients in a home setting
- Follow individualized care plans developed in coordination with physicians
- Administer medications, wound care, and treatments
- Perform assessments, monitor conditions, and update care teams as needed
