Job Overview
We are seeking a dedicated and compassionate Case Manager to join our team. We are a 3.7/3.5 SUD inpatient facility located in Corvallis. The ideal candidate will have a strong background in social work and a passion for helping individuals and families navigate complex systems to achieve their goals. As a Case Manager, you will play a crucial role in providing support, resources, and advocacy for clients facing various challenges, including housing instability, behavioral health issues, and workforce development.
Duties
- Conduct comprehensive assessments to identify client needs and develop individualized service plans.
- Provide ongoing support and case management services to clients, ensuring they have access to necessary resources.
- Collaborate with community organizations, government agencies, and service providers to facilitate client referrals and services.
- Monitor client progress and adjust service plans as needed to ensure optimal outcomes.
- Offer crisis intervention and support during critical situations.
- Educate clients about available programs such as Section 8 housing assistance, childcare options, and Medicare benefits.
- Assist clients with workforce development initiatives, including job training and mentoring opportunities.
- Maintain accurate documentation of client interactions, progress notes, and case files in compliance with agency policies.
Qualifications
- Bachelor’s degree in Social Work or a related field; relevant experience of at least two years may be considered in lieu of formal education.
- Experience in behavioral health, or addiction counseling is preferred.
- Knowledge of community resources related to housing assistance, healthcare services, and workforce development.
- Strong interpersonal skills with the ability to build rapport with diverse populations.
- Excellent communication skills for effective advocacy on behalf of clients.
- Ability to work independently as well as collaboratively within a team environment.
- Proficient in crisis intervention techniques and problem-solving strategies.
- Familiarity with relevant regulations and policies affecting clients’ access to services.
