What are the responsibilities and job description for the Program Coordinator position at Beaufort County South Carolina?
The Human Services Program Coordinator is responsible for coordinating and facilitating Beaufort County Human Services Department programs and processes in accordance with local, state, and federal regulations. This position performs micro-level (frontline) social work for service populations, including conducting assessments, participating in multidisciplinary team meetings, coordinating interventions, and providing ongoing support; prepares routine reports outlining successes, challenges, and barriers; ensures all programs and processes are delivered in accordance with established policy and with fidelity to their respective models and best practices.
The essential functions listed below are those that represent the majority of the time spent working in this class. Management may assign additional functions related to the type of work of the class as necessary.
- Facilitates the delivery of services by coordinating with various stakeholders, including social workers, healthcare providers, and community organizations
- Organizes and participates in meetings with multidisciplinary teams to discuss cases, develop intervention plans, and ensure cohesive service delivery
- Establishes and maintains monthly staffing schedules for collaborative meetings conducted by the department
- Performs initial and ongoing assessments of service populations to identify needs, strengths, and appropriate interventions
- Maintains accurate and confidential records of client interactions, assessments, and interventions in accordance with privacy laws and organizational standards.
- Provides direct support to clients through ongoing case management to address their needs and goals.
- Acts as an advocate for clients, ensuring they receive appropriate services, and supports them in overcoming barriers to access
- Performs other related duties of a similar nature or level
MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS:
- Requires a Bachelor’s degree in Social Work, Public Health, Human Services, Psychology, Sociology, Community Development, or a related field.
- Minimum of two years of related experience or an equivalent combination of education, training, and experience.
SPECIAL CERTIFICATIONS AND LICENSES:
- Must possess and maintain a valid state driver’s license with an acceptable driving history.
