NOW HIRING: Director of Social Services
Responsibilities Director of Social Services:
- Develop and implement social service programs to meet the individual needs of residents.
- Conduct psychosocial assessments and develop care plans in collaboration with interdisciplinary teams.
- Provide counseling and support to residents and their families to address social, emotional, and psychological concerns.
- Coordinate discharge planning and community resources for residents transitioning to other levels of care or back to the community.
- Maintain accurate and up-to-date resident records and documentation in compliance with state and federal regulations.
- Collaborate with staff, residents, and families to create a supportive and inclusive environment within the nursing home.
Qualifications:
- Bachelor's or Master's degree in Social Work or related field from an accredited institution.
- State licensure or certification as a Social Worker (LSW, LCSW, or equivalent) required.
- Experience in social work, preferably in a long-term care or nursing home setting.
