What are the responsibilities and job description for the Community Health Worker position at Bellin Health?
Location: 744 S Webster Ave, Green Bay, WI 54301
Shift Hours:
Limited Term 1.00 FTE (40 hours/week), Monday - Friday 8am-4:30pm
Job Description: The Community Health Worker through partnership with COMSA (Community Service Agency Inc) and Bellin Health will work under the DHS grant for Mobilizing Health Equity within the Brown County Somali/Refugee population. With focus to identify social determinant of health data to reduce health disparities and enhance cultural awareness in the health system. The CHW will work strategically with the two organizations to identify community level barriers and cultivate additional partnerships within Brown County to bridge gaps for the targeted population. Goal is to aid in resettlement of the Somali population by advocating for patient needs, aligning patients to Bellin Health and appropriate community resources, performing patient outreach, education, and identifying opportunities to enhance care coordination. Other responsibilities include maintaining accurate health data on Somali population, attend DHS required reporting meetings, and develop outcome measures to reduce health disparities.
Qualifications:
Education : High School Diploma
Certification/Registration/Licensure : Valid Wisconsin driver’s license and auto insurance required
Experience : One year recent experience in a health care or human service related fields. Familiar with community resources and COMSA mission. Somali speaking.
