At Bennington Home Health, we are dedicated to providing exceptional care to our clients and their families. We believe in a personalized, compassionate approach to home care, and we're looking for an organized, detail-oriented Scheduling Coordinator to assist our team on the field.
This is a part-time role and hours will range anywhere from 15-25 hours per week.
Responsibilities:
- Conduct initial patient assessments during start-of-care visits, evaluating physical, functional, cognitive, and psychosocial status.
- Develop and document individualized care plans based on patient needs, goals, and physician orders.
- Collaborate with patients, families, and on-the-field team members to coordinate care and ensure a smooth transition to home health services.
- Perform medication reconciliation, review home safety, and educate patients and families on their care plan, medical equipment, and treatments.
- Ensure all required documentation is completed accurately and submitted promptly in compliance with regulatory and agency standards.
- Serve as the primary point of contact for patients and families during the start-of-care process, addressing concerns and answering questions.
- Identify and report any significant changes in patient condition to the appropriate healthcare providers.
- Follow agency protocols, industry standards, and state and federal regulations for home health care services.
Qualifications:
- Experience: Minimum 1 year in a customer service role, preferably in a home health role
- Education: High school diploma or equivalent required
- Must be legally authorized to work in the U.S.
