Best Home Health has been serving patients in Northern Louisiana for over 35 years. We use a multi-disciplinary approach in providing care to our patients, combining skilled nursing, home health aides, physical therapists, occupational therapists, and speech therapists, to ensure that our patients receive the best care for their individual needs. Our priority is compassion and care, to both our patients and our team.
As the Office LPN, you work alongside the Clinical Manager and other members of the patient care team in a collaborative approach to manage patient care in the best way suitable for each individual. This position is largely an office position; however, there are times in which the Office LPN also provides in-home care to patients.
Responsibilities:
- Utilize nursing knowledge and skills to deliver excellent individualized care to patients within the comfort of their homes.
- Gather data, evaluate patients, and maintain patient files, records, and other information in a professional manner, following the correct policies and procedures.
- Coordinate communication effectively with all parties involved in the patients’ care: caregivers, family, physicians, other home health team members, etc, to coordinate optimal care.
- Ability to adapt to change with a positive attitude.
- Coordinates and communicates between field staff and physician offices.
- Provide direction and coordinates schedules for HHAs.
- Receives referrals and directs them in the appropriate manner to home office to generate admit orders.
- Maintains the referral log.
- Follow-up daily on hospitalized patients.
- Assists with answer the phone, as needed.
- Participate in multi-disciplinary case conferences and staff meetings, sometimes functioning as the facilitator.
What you bring to the table:
- Graduate of an accredited school of practical/vocational nursing
- One year experience as an LPN
- Must hold a current state or compact state license
- Ability to lead effectively
- Time Management Skills
- Organizational Skills
- Demonstrate good judgment in patient care
- Excellent communication skills
- Dependable transportation, driver’s license, and car insurance
- CPR Certified
- Willingness to serve others
Job Type: Full-time
Pay: $20.00 - $22.00 per hour
