Director of Case Management
Position Overview
The Director of Case Management will provide strategic support to Valley View Medical Center by ensuring high-quality, cost-effective patient care through the management of case services, utilization review, and discharge planning. This senior-level role reports directly to the Chief Nurse Officer and is responsible for assessing, planning, implementing, and evaluating patient needs to enhance care continuity, quality, and satisfaction.

Compensation
- Salary: $118,000 - $150,000 (USD), confidential
- Signing Bonus: $20,000 with a $5,000 relocation package for a 3-year commitment
Key Responsibilities
- Lead case management functions in alignment with the organization’s mission, vision, and strategic goals.
- Develop and implement goals, standards, and objectives to promote cost-effective and continuous patient care.
- Oversee the integration of social services, case management, and discharge planning with other departments and external agencies.
- Conduct medical record reviews to ensure adherence to medical staff, TJC, CMS, and state standards.
- Act as a patient advocate, addressing adverse occurrences, providing staff education, and managing resource utilization.
- Ensure services are utilized appropriately from admission through discharge, coordinating interdisciplinary rounds and optimizing resource use.
- Develop effective physician relationships, guiding cost management and patient outcome strategies.
- Direct policy and protocol development, ensuring social services support patients and families in managing the impact of illness.
Requirements
- Must-Haves
- Bachelor of Science in Nursing or higher
- BLS certification upon hire
- Arizona or Compact State RN License
- Minimum 2 years as a Director of Case Management in a critical care hospital
- Nice-to-Haves
- CCM or ACM Certification
- Master’s degree
- 5 years in case management leadership or similar roles in critical care hospital management
Job Type: Full-time
Pay: $134,320.00 - $150,000.00 per year
