Job Summary
We are seeking a compassionate and dedicated Home Care Nurse to provide high-quality care to individuals in their homes. The ideal candidate will have experience working with diverse populations, including those with developmental disabilities, autism, and memory care needs. This role requires a strong commitment to patient observation and the ability to create and implement effective care plans tailored to each individual’s unique requirements.

Responsibilities

• Deliver personalized nursing care to clients in their homes, ensuring comfort and safety.
• Develop and implement comprehensive care plans based on individual patient assessments.
• Monitor patients’ health status through regular observation and documentation of vital signs.
• Assist clients with daily living activities, including cooking, cleaning, and personal hygiene.
• Provide support for individuals with developmental disabilities and autism, fostering independence while ensuring safety.
• Administer medications as prescribed and educate clients and families about medication management.
• Collaborate with family members and other healthcare professionals to coordinate care effectively.
• Maintain accurate records of patient progress and communicate any changes in condition promptly.

Qualifications

• Valid nursing license (LPN or RN) in the state of practice.
• Experience in assisted living or home care settings is preferred.
• Knowledge of care plans and patient observation techniques is essential.
• Certification in CPR and First Aid is required.
• Strong interpersonal skills with the ability to work effectively with patients, families, and team members.
• Experience working with individuals with developmental disabilities or autism is a plus.
• Ability to perform light housekeeping tasks as needed for client care.

Job Types: Full-time, Part-time

Pay: $14.00 - $16.00 per hour

Expected hours: 10 – 40 per week
