Job Summary

Responsibilities
- Deliver personalized nursing care to clients in their homes, ensuring comfort and safety.
- Develop and implement comprehensive care plans based on individual patient assessments.
- Monitor patients’ health status through regular observation and documentation of vital signs.
- Assist clients with daily living activities, including cooking, cleaning, and personal hygiene.
- Provide support for individuals with developmental disabilities and autism, fostering independence while ensuring safety.
- Administer medications as prescribed and educate clients and families about medication management.
- Collaborate with family members and other healthcare professionals to coordinate care effectively.
- Maintain accurate records of patient progress and communicate any changes in condition promptly.
Qualifications
- Valid nursing license (LPN or RN) in the state of practice.
- Experience in assisted living or home care settings is preferred.
- Knowledge of care plans and patient observation techniques is essential.
- Certification in CPR and First Aid is required.
- Strong interpersonal skills with the ability to work effectively with patients, families, and team members.
- Experience working with individuals with developmental disabilities or autism is a plus.
- Ability to perform light housekeeping tasks as needed for client care.
