Better Morning emerged as an outpatient behavioral health practice in Ashburn, VA in the year of 2014. In addition to providing counseling from the Ashburn office, Better Morning started off as a certified provider for intensive in home and community-based services (IHCBS), for at-risk youth in District of Columbia. In August of 2017, Better Morning was certified as a Core service agency (CSA) by DC Department of Behavioral Health. Better Morning founder’s passion for at-risk youth and their family were the motivation to keep expanding the evidenced based programs to meet the need of the underserved population.

Job Responsibilities of a Case Manager for Transitional Youth Housing Program :

  • Conduct comprehensive assessments of participants’ needs, including housing, education, employment, mental health, and life skills.
  • Develop individualized service plans with short- and long-term goals tailored to each youth’s circumstances.
  • Evaluate eligibility for services and ensure appropriate documentation is collected.
  • Connect youth with essential services, including healthcare, mental health counseling, education programs, and job training.
  • Facilitate access to housing benefits, public assistance programs, and community resources.
  • Work with external service providers to ensure continuity of care.
  • Provide regular one-on-one sessions to review progress toward goals and adjust service plans as needed.
  • Monitor participants’ well-being, offering guidance and support in areas like budgeting, time management, and conflict resolution.
  • Offer crisis intervention, including de-escalation and referrals to emergency services, if needed.
  • Empower youth to develop self-sufficiency skills, advocating for their needs and interests with service providers.
  • Assist participants in building social networks and support systems to promote long-term stability.
  • Help youth transition into permanent housing, employment, or educational programs.
  • Work closely with other staff members, such as engagement specialists, counselors, and housing coordinators, to ensure a holistic approach to care.
  • Attend case conferences, team meetings, and trainings to collaborate on best practices and program development.
  • Partner with community organizations and agencies to enhance service delivery.
  • Maintain accurate, up-to-date case files, including service plans, case notes, and progress reports.
  • Track program outcomes and ensure compliance with organizational policies and funder requirements.
  • Prepare and submit timely reports on client progress and housing status.
  • Provide input on program development and suggest improvements based on client needs and challenges.
  • Stay informed about best practices, policies, and resources related to youth housing and homelessness services.
  • This role requires excellent interpersonal, problem-solving, and organizational skills, with an emphasis on empathy, cultural competence, and trauma-informed care. A case manager must be comfortable working independently and in a team environment to guide youth toward stability and independence.

    Minimum Qualifications :

    BS in Health Science OR 3 years’ experience working with youth and / or homeless population.

    Job Type : Full Time - Contract

    Pay : $32.00 - $35.00 per hour

    Schedule :

    8-hour shift, Day shift, Monday to Friday

    Ability to commute / relocate :

    Washington, DC 20020 : Reliably commute or planning to relocate before starting work (Required).
