LVN Nurse

Overview
We seek a dedicated and compassionate Licensed Vocational Nurse (LVN) to join our healthcare team. The ideal candidate will have a strong background in patient care, particularly in HOME setting. This role requires excellent communication skills, a thorough understanding of anatomy and geriatrics, and the ability to work effectively within a team to provide high-quality care to our patients.

Duties

• Provide direct patient care under the supervision of registered nurses and physicians.
• Administer medications and treatments as prescribed, ensuring compliance with safety protocols.
• Assist with patient assessments and document vital signs accurately.
• Maintain a clean and sterile environment by adhering to infection control protocols.
• Collaborate with interdisciplinary teams to develop and implement individualized care plans.
• Educate patients and their families on health management and disease prevention.
• Utilize electronic health record systems such as Hospice MD for accurate documentation.
• Participate in training new staff members in sterile processing techniques and patient care procedures.

Requirements

• Valid Licensed Vocational Nurse (LVN) license in the state of practice.
• Experience in clinical settings, nursing homes, or assisted living facilities is preferred.
• Strong understanding of anatomy, geriatrics, and basic nursing principles.
• Ability to lift patients safely using proper techniques.
• Excellent interpersonal skills with the ability to communicate effectively with patients, families, and healthcare team members.
• Proficiency in electronic medical records systems; familiarity with Hospice Md is advantageous.
• Commitment to providing compassionate care while maintaining patient confidentiality.
• English needed, Spanish speaking is preferred
