Job Title: Social Work Director
Bishop Rehabilitation & Nursing Center seeks a skilled professional to lead our Social Services Department.
Duties:
- Develop and implement departmental policies and goals, ensuring alignment with facility objectives.
- Conduct comprehensive assessments, create personalized care plans, and facilitate resident adjustment to our community.
- Supervise department staff, providing guidance and support to ensure exceptional service delivery.
- Collaborate with interdisciplinary teams to develop discharge plans at the onset of admission, ensuring seamless transitions.
- Foster community connections through occasional outreach initiatives.
- Maintain in-depth knowledge of various concepts, practices, and procedures essential to social work in long-term care settings.
- Possess a high degree of creativity and latitude, relying on extensive experience and sound judgment to drive goal accomplishment.
Requirements:
- Minimum 2 years of management experience in Long-Term Care (LTC) settings, preferably with a strong background in social work.
- Licensed Social Worker (LSW) or Master's in Social Work (MSW) degree required.
- Thorough understanding of applicable State and Federal guidelines governing LTC facilities.
- Exceptional interpersonal and leadership skills, with the ability to motivate and inspire team members.
