Transition Coordinator – Bachelor’s req’d

Full-Time: Mon-Fri, 8:30am - 3:30pm
Salary: $65,000 - $78,929/yr
Brookville Center for Children's Services provides comprehensive services for children 3-21 years of age with intellectual and other disabilities, traumatic brain injury, and autism. Our mission is to educate and support children of all abilities.
We are currently seeking a Transition Coordinator to manage and oversee all Transition Programs: classroom-based, center-based, and community-based locations.
In this role, you will:
- Coordinate transition process for adolescent students with classroom teacher and social workers.
- Organize activities for adolescents 15 to 21 years of age to aid in successful transition to adult service.
- Coordinate meetings with social workers for families of each transition age group level two times per year.
- Establish age-appropriate transition goals for all adolescent students.
- Supervise the art/transition teacher and aide.
- Conduct monthly meetings with Transition staff regarding the progress of students and appropriate goal implementation.
- Schedule staff assignments.
- Review existing off-site prevocational/transition placements to ensure each student is appropriately assigned.
- Review and correct all art/transition annual review reports.
- Meet with Transition staff on a weekly basis.
- Participate in the performance appraisal process for assigned staff.

We require:
- Bachelor of Science Degree AND currently enrolled in a Master’s Special Education Program
- Minimum of one (1) year of special education teaching experience, with an emphasis on adolescent students with developmental disabilities
- NYS Valid and Qualified Driver’s License (maintained throughout employment)
- Familiarity with the transition process for the developmental disabilities adult services program
- Ability to create appropriate activities related to transition planning
- Physical aspects may include but are not limited to lifting and transferring of a student
