The Community Support Services (CSS) Coordinator plays a crucial role in supporting the CSS Manager in delivering trauma-focused, culturally aware, and community-based direct services to individuals, youth, and families impacted by community violence in the service areas of Austin, Lawndale, Garfield Park, Humboldt Park, Logan Square, Belmont Cragin, and Hermosa. The CSS Coordinator is responsible for the administrative and operational functions of the program, including supporting wellness advocate teams, ensuring grant compliance, and promoting community engagement. The CSS reports directly to the Manager of Community Support Services (MCSS).
Responsibilities are but not limited to :
Team Coordination and Supervision :
- Set up weekly canvassing for each team member.
- Provide weekly task supervision to the team of case managers.
- Conduct daily team huddles to provide ongoing support and address client needs.
- Support Wellness Advocates in maintaining timely, accurate, and compliant documentation within the Apricot system to meet program and grant requirements.
- Support formalized organizational-wide care coordination efforts
- Participate in regular departmental case coordination meetings to ensure collaboration amongst internal and external program administrators
