The Lewelling Shelter and Drop-In Center, set to open in March 2025, will serve chronically homeless individuals. The center comprises 29 interim - shelter rooms and offers drop-in services (showers, laundry, meals, meetings and individual meetings) five days weekly.
POSITION SUMMARY: The Case Manager provides case management and supportive services to drop-in center guests, people experiencing homelessness. The case manager meets guests where they are and supports them to achieve their goals. Building Futures services are housing-focused; the Case Manager works with and supports guests on developing their long-term housing plans. The Case Manager also connects guests to appropriate supportive services, including medical treatment, mental health treatment, counseling, and permanent housing.
ESSENTIAL FUNCTIONS
- Develop and maintain accurate client files with all documents, verifying program requirements have been met
- Gather and maintain confidential client records
- Provide clients with internal and external agency referrals as needed Assess and refer clients to resources and programs including life skills, employment, mental health, transportation, and education programs, and help clients successfully access these resources
- Conduct housing problem solving and case management with a focus on immediate needs.
- Maintain confidentiality of all visitors and clients
- May be required to drive a company vehicle to transport clients. For insurance purposes, drivers must be at least 25 years of age, have a valid CA Drivers License, and must pass a Motor Vehicle Report
- Other duties as assigned
QUALIFICATIONS
The preferred candidate will have a bachelor’s degree in a related field, and will have at least two years of relevant experience working with people experiencing homelessness. To perform the job successfully, demonstrate:
- Positivity and passion for ending homelessness
- Strong professional boundaries and interpersonal, analytical, and time management skills
- Professional verbal and written communication skills in English
- Ability to work independently and as part of a team; meet or exceed goals and produce positive results with minimal supervision
- Ability to handle multiple cases while maintaining strategic thinking and
- Ability to work well with people from diverse backgrounds
- Competent in required job skills and knowledge; able to learn and apply new skills, keep abreast of current developments, and use resources effectively
- Able to adapt to new technologies, troubleshoot technological problems, use technology to increase productivity, and keep technical skills up to date.
- Skilled at handling difficult or emotional customer situations
- Able to express ideas and thoughts verbally and in writing; exhibiting listening and comprehension; keeps others adequately informed using appropriate communication methods
- Adaptable to changes in the work environment while managing competing demands; able to deal with frequent change, delays, or unexpected events
- Demonstrates accuracy and thoroughness; looks for ways to improve and promote quality of service; applies feedback to improve performance
- Works as a team player with strong interpersonal and multi-tasking skills, initiative and flexibility.
