What youll do:
As the Social Services Assistant, you are responsible for:
- As the Social Service Assistant, you will work with residents and families to assist in providing services and ensure they have access to the services to meet their needs.
Duties:
- Assisting in the development and implementation of client care plans.
- Maintaining accurate and up-to-date client records and case files.
- Conducting preliminary client interviews and assessments.
- Coordinating with other service providers and agencies to ensure comprehensive care.
- Providing practical and emotional support to individuals and families.
Qualifications
- At least 1 year of Long-Term Care experience is preferred.
- At least 1 years of experience in customer service.
- Excellent judgment, analytical, and interpersonal skills, including written and verbal communication skills to effectively communicate with all departments.
- Excellent note taking skills.
- Knowledge of Medical Terminology required.
- Ability to work independently and within cross-functional teams.
- Excellent calendar management skills, including the coordination of complex executive meetings.
- Ability to research, analyze data and solve problems effectively.
