Job Title: Resource Specialist
Location: Remote – Columbus/Central Region Area Highly Preferred

About the Role:
We are seeking a dedicated Resource Specialist to perform member outreach and address social determinants of health needs. This field-based role involves remote work, with a preference for candidates in the Columbus/Central Region area.
Key Responsibilities:
- Conduct member outreach to identify and resolve social determinant of health needs.
- Utilize strong customer service skills and motivational interviewing techniques.
- Work within electronic health record (EHR) systems to manage member information (experience preferred).
- Coordinate with community-based organizations and resources to provide essential services to members.
- Track and assess progress based on assigned tasks and member needs.
Performance Expectations:
- Meet target metrics related to closed-loop resolution rates and successful completion of member needs assessments.
- Effectively manage daily assigned tasks and document progress within health records.
First-Day Onboarding:
- Gain access to Buckeye Health Plan systems.
- Meet the team and hiring manager.
- Receive an overview of role responsibilities and review training/resources.
- Participate in an initial meeting to discuss expectations and workflow.
Qualifications:
Required Skills & Experience:
- CHW Certification (any recognized CHW education/program).
- Experience with EHR systems and computer-based documentation.
- Customer service and motivational interviewing experience.
- Experience working with community-based organizations and connecting members to resources.
Preferred Skills & Experience:
- Experience managing difficult interactions and sensitive conversations.
- Prior work as a Community Health Worker (CHW), Community Resource Coordinator, or Public Health Worker.
- Familiarity with Trucare (not required but helpful).
Interview Process:
- One interview conducted via Microsoft Teams.
