Description

Lower Lights Christian Health Center (LLCHC) transforms the overall health of Central Ohio, serving one individual at a time. We are focused on whole-person wellness, available to ALL in Central Ohio who need it, regardless of ability to pay!  In 2019 alone, we served over 12,000 patients - with 40% being uninsured - and totaled 50,000 medical encounters!

Operating out of seven locations, we offer medical care (primary care, dental, vision, OB/GYN, telehealth), behavioral health care, 340B pharmacy, nutritional assistance programs, and more. Working hours are Monday - Friday with occasional Saturday morning coverage. 

We are seeking a Community Health Worker who will collaborate with clinical and medical staff to provide culturally sensitive one-on-one support and guidance to patients and families with social determinants of health (SDH) needs. 

ESSENTIAL JOB RESPONSIBILITIES:

RESPONSIBILITY 1. Referral & Coordination of Care 

  • Addresses identified patient and family SDH needs and improves patient/family’s access to community resources. 

  • In collaboration with clinical staff, links patients/families to essential support services including housing, nutrition assistance, utilities, transportation, and other needs as identified by the patient/family and clinic team. 

  • Assists in conducting follow-up calls to patients and families to confirm linkage with identified community resources and/or services, assesses potential barriers, and aids in navigating resources and services. May accompany patients and families to social service appointments.  

  • Provides appropriate documentation of patient and family interactions in EPIC in a timely manner. 

  • Develops an expertise in making referrals to housing resources, food pantries, utility companies, government agencies, treatment facilities and other internal and external resources, including understanding programs’ eligibility criteria, intake process, etc. 

  • Assists LLCHC clinical staff, supervisor, and organizational partners in the decision-making process for improving linkage of patients/families with community resources.  

  • Collects and report data on program activities. 

  • Reviews and recommends potential resources that are culturally sensitive to the targeted community. 

  • Remains informed of trends, developments, as well as state-of-the-art prevention efforts and strategies, and general information regarding the targeted community. 

Other LLCHC Responsibilities: 

  • Contributes to quality improvement activities of the health center.  
  • Attends and participates in staff meetings.  
  • Assists program management for Women’s Health Department programs as assigned. 
  • Assists with department metrics  
  • Maintain annual Compliance trainings 
  • Maintain professional license if applicable 
  • Other duties within the position scope as assigned

Qualifications


  • Associates required. Bachelor’s degree preferred. 

  • Licensure/Certification: Community Health Worker certification by the Ohio Board of Nursing or currently enrolled in a community health worker certification program. 

  • Experience: 1 year of professional experience with a social service agency, community-based program, or health-related institution. *Experience working in maternal/infant care management is required. 

  • Effective customer service, communication, and interpersonal skills required for interaction with patients, families, and community providers.  

  • Ability to work as a part of a team, with diverse populations, (i.e. racial/ethnic, socioeconomic and educational backgrounds), and with healthcare providers in clinical settings.  

  • Strong attention to detail and organizational abilities required. Sound decision making skills, with the ability to maintain highly confidential information.  

  • Moderate computer skills required (familiarity with Microsoft Word and Excel, required). 

  • Ability to prioritize tasks, problem solve, and work autonomously. 

  • Ability to travel and work flexible hours, which may include some evenings and weekends.
