What are the responsibilities and job description for the Case Manager Mobile Crisis Team position at Capital Area Human Services?
The Mobile Crisis Team B-Case Manager provides targeted case management services which is one of the nine core services provided by CCBHCs. Coordinates admissions, discharges, and follow-up services. Collaborates with clients/families to identify and achieve goals as defined by the persons served. Obtains and prepares social histories of clients for review by treatment team and for use in referrals to other agencies. Manages caseload by scheduling appointments, evaluating client progress toward service plan goals, and making recommendations to the treatment team. Serves as liaison between the client, family, and other community agencies. Assists in the procurement of resources to meet clients' needs such as food/water, clothing, shelter/housing, transportation, financial assistance, income through benefits and/or employment, and medical equipment. Initiates and maintains communication with federal, state, and local agencies to bring applications for support services to successful completion. Documents services provided to clients in the electronic health record.
