The Director of Social Services plays a critical leadership role within our Hospice & Palliative Care organization. This individual will oversee all social work operations, ensuring compliance with state and federal regulations, providing direct support to patients and families when needed, and managing the day-to-day activities of the Social Services team. The ideal candidate will be highly organized, compassionate, and dedicated to supporting the needs of patients, families, and social workers alike.
- Manage, mentor, and supervise all social workers within the organization.
- Monitor compliance with state and federal guidelines, ensuring all documentation and practices meet regulatory standards.
- Conduct regular audits of patient care plans and social worker documentation.
- Respond to urgent needs for social visits due to missed appointments or call-offs.
- Perform occasional field visits to provide direct support to patients and families, addressing psychosocial needs and ensuring quality care.
- Lead regular team meetings to review goals, share updates, and provide training on best practices and compliance standards.
- Develop and implement policies and procedures to improve the quality and efficiency of social services.
- Address performance issues and provide feedback to ensure continuous professional development within the team.
- Work closely with other departments, including nursing, administrative staff, and medical teams, to ensure a cohesive approach to patient care.
- Participate in interdisciplinary team meetings to contribute to the development of comprehensive care plans.
- Serve as an advocate for patients and families, addressing complex psychosocial needs and ensuring access to necessary resources and support.
- Educate patients and families about hospice and palliative care services, bereavement programs, and available community resources.
- Analyze and report on the effectiveness of social service programs, recommending improvements as needed.
- Stay current with changes in regulations and best practices, ensuring compliance and quality improvement.
- Oversee the onboarding, training, and credentialing of new social workers.

Pay: $60,129.00 - $64,988.00 per year
