Job Summary
We are currently seeking a full-time PCA/CFSS Program Coordinator to join our team.
In Minnesota, Personal Care Assistance (PCA) service is being replaced with a new program called Community First Services and Supports (CFSS). As the newest self-directed option, CFSS is designed to offer Participants more choice, control, and flexibility with the services and supports needed to help them live and work in the community.
This role is critical to ensuring a smooth transition for our clients and will involve working with Participants with varying levels of needs throughout the transition period. Once the state completes the full transition, this position will become exclusively focused on the CFSS program.
Key Responsibilities
- Provide initial and ongoing training to Workers, including explaining, coaching, and demonstrating tasks, as well as observing and evaluating Worker competency.
- Ensure service delivery meets high standards of quality and compliance.
- Assist Participants and their families with transitions from PCA to CFSS services.
- Work closely with other team members, including other coordinators and management, to ensure seamless service delivery.
- Stay informed about CFSS program updates and changes, attending meetings and training sessions as needed.
- Complete intake, annual, and ongoing service requirements for PCA and CFSS programs.
- Maintain accurate and thorough documentation as required by DHS and our company.
Qualifications
Candidates must meet the following requirements:
- Holds bachelor’s degree in a related field.
- Has at least 1 year of supervisory and/or worker training experience, preferably in home care.
- Own a reliable vehicle with a valid driver’s license, a clean driving record, and auto insurance.
- Successfully pass a background study with the Minnesota Department of Human Services.

Pay: $52,000.00 - $55,000.00 per year
