Home Health Care Coordinator

About the Role:

This exciting opportunity involves providing daily supervision of nurses and home health aides, ensuring quality of service and providing coaching support to new staff. The ideal candidate will have at least 3 years of experience as an RN, with a strong background in pediatric care and trach, vent, g-tube management.

Key Responsibilities:

1. Supervise nurses and home health aides to ensure quality of service.
2. Identify client needs and evaluate appropriateness of home care.
3. Conduct in-home supervisory visits and maintain confidentiality of medical records.
4. Assist with orientation of new professional staff and provide trainer sessions for peers and field staff.
5. Participate in formal and informal problem-solving processes to meet EMR computer education needs.

What You'll Need:

• Active and current Registered Nurse (RN) license in the state of Pennsylvania.
• At least 1 year of home health care or equivalent experience required.
• Strong knowledge and experience in pediatric care, trach, vent, g-tube management.
• Excellent communication and customer-service skills.
• Proficiency in Microsoft Office applications, electronic medical records, and quick-to-learn proprietary software package.
