At CareAparent you will work with other passionate individuals focused on giving the best care possible for our clients. The Licensed Practical Nurse (LPN) is responsible for providing one-on-one skilled nursing care to clients in their homes, including medication set-ups and management. This position will have the opportunity to collaborate with physicians, clients, their families, and CareAparent’s Home Health interdisciplinary team. Our Case Management model ensures you will receive timely training and support to help you provide the best care possible.
Pay range: $27-35 hourly
Further duties include:
- Performing skilled care functions in the home, including medication management, medication setup, treatments, and procedures
- Documenting visits, orders, actions, and conditions
- Monitoring client conditions, reporting changes to the Clinical Manager or DON
- Performing client assessments as necessary
- Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing electronic medical records on a state-of-the-art touchpad tablet

If YOU have the following, CareAparent wants to hear from you:
- Current Minnesota license as a Licensed Practice Nurse.
- Graduation from an accredited and approved nursing program.
- Ability to work independently and manage time effectively.
- Ability to work on-call for rotating evenings, weekends, and/or holidays, as required.
- Strong interpersonal skills.
- Excellent verbal and written communication skills.
- Solid computer skills: prior experience with electronic medical records (EMR) preferred.
- Valid driver's license, reliable transportation, and ability to travel to cases as assigned.
