We are seeking a caring and dedicated Home Health Aide to join our team at CareConnect. As a HHA, you will play a vital role in providing personalized care to patients in their homes, enabling them to live safely and independently.
Key Responsibilities:
- Provide personal care to patients in their homes
- Assist with mobility and transfers
- Administer medications as directed by healthcare professionals
- Monitor vital signs and report any changes or concerns to medical personnel
- Provide emotional support and companionship to patients and their families
Requirements:
- Certified Personal Care Assistant (PCA) or Certified Home Health Aide (HHA)
- Understanding of HIPAA regulations and patient confidentiality
- Ability to cook simple meals and perform light housekeeping duties as needed
- Satisfactory completion of New York State Health Department-approved HHA Training Program or equivalency
- Demonstrated adequate literacy and simple arithmetic skills
