Job Description
- Deliver high-quality care to patients in their homes, including assistance with daily living activities and mobility support.
- Collaborate with healthcare professionals to develop and implement individualized care plans.
- Monitor patient progress and report any changes or concerns to medical personnel.
- Provide emotional support and companionship to patients and their families.
Requirements
- Certified Personal Care Assistant (PCA) or Certified Home Health Aide (HHA)
- Understanding of HIPAA regulations and patient confidentiality.
- Ability to cook simple meals and perform light housekeeping duties as needed.
- Satisfactory completion of New York State Health Department-approved HHA Training Program or equivalency.
- Demonstrated literacy and arithmetic skills.
- Age 18 or older.
