Registered Nurse Case Manager

Responsibilities:
- Conduct patient assessments to determine individual needs and develop appropriate care plans
- Coordinate and collaborate with healthcare professionals to ensure the delivery of high-quality patient care
- Monitor patient progress and make necessary adjustments to treatment plans
- Provide education and support to patients and their families regarding medical conditions, treatment options, and available resources
- Maintain accurate and up-to-date medical records
- Perform routine patient care tasks such as taking vital signs, administering medications, and assisting with activities of daily living
- Advocate for patients' rights and ensure their needs are met within the healthcare system
- Collaborate with primary care providers to ensure continuity of care
- Participate in interdisciplinary team meetings to discuss patient progress and develop effective care strategies
Qualifications:
- Valid Registered Nurse (RN) license in Ohio

- Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare professionals
- Ability to work independently as well as part of a collaborative team
