Key Responsibilities
- Conduct initial and comprehensive nursing evaluations to determine primary focus of care and develop an individualized plan of care within state-specific guidelines.
- Provide and direct provisions of nursing care to patients in their homes, as prescribed by physicians and in compliance with applicable laws and regulations.
- Coordinate total plan of care with other healthcare professionals involved in care and help achieve and maintain continuity of patient care.
