What are the responsibilities and job description for the Program Manager at Day Program position at CARING HANDS TRANSPORT LLC?
Caring Hands adult day program for seniors and adults with special needs, seeks an administrator that is enthusiastic, compassionate, kind, and reliable.
This position will be a combination of direct care responsibilities working one on one with specific clients, developing, and implementing group programs, and activities.
Responsible for directing and overseeing the day-to-day operations of a licensed adult day program (that are licensed and inspected by the State of Colorado).
Plans, organizes, and interprets the policies of the adult day program and transportation department's services, objectives, and standards practices.
Assures compliance with Federal and State regulatory agencies to include, but not limited to, staffing requirements, medical and non-medical services provided, dietary regimens, social activities, staff education, and safety plans, etc.
Responsible to identify potential abuse and neglect cases and report to the regulatory agency, as necessary.
Monitors program services to assure quality of care and to recommend changes in program guidelines as necessary to meet the needs of clients served at the adult day program and by the transportation department.
Recommends a budget to implement stated objectives.
Fosters a professional, healthful environment and an atmosphere of positive employee morale.
Hours: Monday through Thursday from 6:30am to 4:30pm
Essential Duties and Responsibilities:
Must understand and accept that the needs of the client define the parameters of direct care staff duties; the Day Program Director will outline any changes in staff duties as the appropriate for the needs of the client.
Always consult with the Day Program Director in the event of questions regarding a client's Individualized Services and Support Plan (ISSP)
Participate in training provided to learn about developmental develop, best practices, the individual participant's needs and goals, correct documentation, and any other job-related responsibilities.
Collaborate with the client's guardian and/or group day program staff in order to keep open communication regarding the member's progress.
Provide appropriate training in independence, self-care, communication, social skills, recreational and leisure skills, and/or vocational and pre-vocational skills; assist in all areas of programming; member shall never be left alone.
Provide necessary supervision of the client in all aspects of the program when needed.
Document clients' progress appropriately and submit in a timely manner.
Receive supervision and feedback regarding job performance from the Day Program Director
Transport members from residence to program, from program to residence, and to community connections
Ensure that the CO DHS records management and documentation rules and Caring Hands' Policy and Procedure Manual are followed without exception.
Meet with and participate as required/available in all Caring Hands staff monthly for QA/QI staff meetings.
Implement client's Individualized Services and Support Plan (ISSP); assess the status of clients and address all problem behaviors and incidents in the manner outlined in each client's Individualized Services and Support Plan (ISSP)
Provide support in all aspects of life including to not limited to basic self-care and socialization, personal hygiene and grooming, health support, personal assistance, feeding, cooking, laundry, cleaning, transfers, handling of bodily fluids (using universal safety precautions), recreational/vocational/educational activities, nutrition, emotional support, hobbies, etc.
Perform other related administrative duties as assigned.
Coordinate a variety of services from a team approach that can be used for consistent planning for clients in the community.
Monitor progress of the client, reassessing continued need and evaluate the quality of services received.
Ensure clients have viable service plans at all times AND address client's service plan needs immediately.
Secure all initial service authorizations where required, as well as re-authorized prior to the expiration of the authorization.
Report critical incident and ensure client safety in the event of incidents.
Provide on-going contact with clients, families, and case managers.
Demonstrate open and clear communication between clients, families, and case managers -- respond to member/family inquires promptly.
Note all changes in diagnosis or medication in the client's chart in a timely manner.
Work with clients in development of skills related to: Community living, such as housekeeping, shopping, cooking, use of transportation facilities, money management; Personal care such as health care, medication self-management, grooming; social relationships; and use of leisure time.
Educational activities which include assisting the clients in securing needed education services such as adult basic education and special interest courses; and Pre-vocational activities which focus on the development of positive work habits and participation in activities that would increase the participant's self-worth, purpose and confidence; these activities are not to be job specific training.
Monitor operations to ensure that staff members comply with administrative policies and procedures, safety rules, contracts, environmental policies, or government regulations.
Give facility tours.
Assist director and general manager.
Post events on company website and social media sites
Serve as communications hub in the event of an emergency or during weather closures.
Skills and Qualifications:

Must be able to pass a CBI/FBI fingerprint and CAPS background check.
Must be able to complete all trainings provided by Caring Hands within 90 days of hire including CPR and First Aid certification, and QMAP certification.
Must demonstrate competence by exhibiting core skills including technical knowledge, cultural awareness, analytical skills, decision-making, interpersonal skills, communication skills, and clinical skills.
Knowledge of basic human physical/psychological needs and simple health and safety precautions
Must be able to perform each essential duty satisfactorily; reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
High School Diploma or equivalent with at least 2 years of direct care experience at a day program or bachelor's degree in Human Services or related field preferred
Preferred candidate will be at least 25 years of age and have experience teaching or working with individuals with developmental/intellectual disabilities and/or mental health issues and the ability to understand brain injury.
Valid driver's license and clean driving record (no more than 6 points in the last 3 years)
Being fluent in a second language is a plus but not required.
