Caritas Communities is the largest non-profit owner of single-room occupancy (SRO) housing in Greater Boston – serving over 1,000 people annually.
Position Summary:
The Supportive Services Program Manager (SSPM) is a critical role overseeing the Caritas Communities supportive services team, comprised of Resident Service Coordinators (RSCs) and CSP Case Managers (CMs). The SSPM is responsible for ensuring seamless enrollment of eligible residents into the CSP-HI and eventual CSP-JI programs across Caritas Communities properties. This position requires both in-office and field-based work to conduct comprehensive intake assessments, develop service plans, and coordinate with supportive services, property management, and leasing teams to achieve enrollment goals. The ideal candidate possesses a strong knowledge of best practices for working with individuals exiting homelessness using a Housing First framework and a trauma-informed lens. The ideal candidate has significant experience working with and supporting staff working with vulnerable populations.
The CSP-HI program is new to Caritas Communities. This position has excellent potential for growth. We are seeking a highly qualified and motivated individual to take ownership of the advancement and success of the Caritas Communities supportive services program.
Job Duties:
- Work based out of the and in the field at various property sites, ensuring that all new prospects for move-in are vetted for CSP-HI eligibility.
- Collaborate with the Clinical Director, supportive services, property management, and leasing teams to meet enrollment goals.
- Conduct CSP-HI Screening Tool for all Caritas Communities incoming residents who self-identify as homeless.
- Open eligible clients in Ehana, work with CSP Billing Coordinators to verify client Admission/Enrollment date.
- Complete initial documentation (CSP-HI Agreement, Screening Tool, Self-Sufficiency Matrix, and Action Plan.)
- Assist assigned case managers with ongoing documentation and first CSP-HI Service Note.
- Supervise and train CSP case managers and Resident Service Coordinators.
- Monitor Ehana for quality case notes, ensuring that program minimums are met and exceeded with appropriate documentation.
- Monitor Apricot 360 for quality resident encounter forms, ensuring that program minimums are met and exceeded with appropriate documentation.
- Generate and submit CSP bills monthly.
- Carry a caseload of the highest acuity clients/residents (approximately 5).
- Respond to all program-related audits.
- Provide additional support to the supportive services team as business needs arise.
Qualifications and Skills:
- Bachelor's degree with 2-5 years experience in housing and homeless services; Master's in Social Work, Public Health or similar discipline preferred.
- 2-5 years of experience working with an intensive case management model (CSP-HI/ CSPECH preferred)
- 2-5 years working with people experiencing homelessness.
- 2-5 years in a management or supervisory role.
- Experience with Motivational Interviewing and Trauma Informed Care is preferred. A new hire would be expected to complete internal training in both Motivational Interviewing and Trauma Informed Care within 90 days of hire.
- Proficiency with Microsoft Office Suite.
- Proficiency with EMR systems, preferably Ehana.
- Must possess a valid driver's license and access to a reliable vehicle.
- Ability to establish good rapport with resident clients.
- Effective communication skills with internal and external stakeholders.
- Ability to collaborate with other departments.
- Time management skills and the ability to prioritize tasks and meet deadlines.
