General Duties:

Provides for the safety and highest quality of life of designated program participants
Provides work specific oversight to individuals and/or groups of participants
Complies with all agency policies and procedures
Participates in meetings as assigned
Accurately documents participants attendance, progress, and behaviors
Presents self in a professional manner

Demonstrates knowledge of designated program participant’s strengths, needs, preferences, and desires
Successfully completes all DDA and agency required trainings
Specific Duties:
- Registers for and attends all trainings as required.
- References Nursing Supports calendar to ensure timely participation and completion of Plan of Care Trainings, competency reviews, remediation, etc…
- Contributes to effective and efficient communication between the team
- Accurately communicates needs of designated program participants to DSC
- Participates in various team meetings as scheduled
- Provides services related to the provision of program services as outlined in the Person- Centered Plan (PCP)
- Provides supports, based on the PCP, in a variety of settings, including residential during emergency program closures and as assigned
- Encourages and facilitates involvement and social networking in greater communities
- Accurately documents services provided in the format approved by the agency
- Documents observations/interventions according to agency procedures
- Monitors behavior and activities of program participants in accordance with documented individual need
- Administers/Monitors medications and treatments as delegated by RN CM/DN
- Utilizes behavior management techniques and other strategies to promote the highest quality of life in the least restrictive manner
- Encourages skill development of designated program participants
- Monitors and instructs participants regarding desired activities and promotes independence
- Assists designated program participants with generally acceptable behavior skill development
- Offers instructional and experiential opportunities to develop desired skills
- Advocates for participates to experience competitive employment when indicated
- Provides transportation and support to designated program participants as needed, such as healthcare appointments and events in the greater community
- Facilitates and supports access to, and use of, public transportation options
- Completes program chores as assigned
- Follows all doctors’ orders and reports any concerns to nurse on duty
- Accurately completes Notable Incident Reports and Maintenance Repair Requests
- Participates in the functions of agency committees and special projects as assigned
Job Type: Full-time
Pay: $15.50 - $16.43 per hour
