Job summary statement : This is a grant funded position through the Substance Abuse Mental Health Services Administration (SAMSHA). The purpose of the Tu Bienestar project is to increase entry to mental health and addiction treatment services, improve wellbeing, and reduce HIV risk-behaviors of Latina women and men. S / he will conduct outreach and educational activities to engage clients in Casa Esperanza's Familias Unidas Outpatient Program. S / he will work across Casa's four programs to provide case coordination and care management for individuals at risk for HIV. The HIV Outreach Worker / Case Manager will ensure the Tu Bienestar program is delivering and documenting the provision of high quality services in compliance with funder requirements, and agency and accreditation standards. S / he will ensure that benchmarks, timelines, goals and objectives are being met and reported as required.

Essential Functions :

Tu Bienestar :

  • Implement and oversee client recruitment / outreach strategies that achieve intake and follow-up goals of the program.
  • Coordinate admissions and intake for female clients to the Familias Unidas Outpatient Program, including liaising with internal and external programs.
  • Conduct HIV / STI / Hep A, B, C individual and group prevention sessions.
  • Conduct HIV CTR (Counseling / Testing / Referral) during regular walk-in hours in our medical clinic on campus as well as during street and peer provider outreach, and counseling and testing events and coordinate other Infectious Disease screening and testing with Boston Health Care for the Homeless medical staff.
  • Provide health navigation services including obtaining health insurance, choosing a Primary Care Provider, and supported referral and translation for appointments as needed.
  • Refer clients to appropriate outside services such as : medical, dental, benefits programs such as SSI, HDAP, SNAP, housing, financial, legal, education, training, employment, life skills, peer support and other services, and peer supports such as AA and NA groups.
  • Provide care coordination for services delivered both on and off campus.
  • Coordinate client transportation as needed.
  • Work closely with Casa's Multi-disciplinary Team (MDT) to ensure effective care management and meet Tu Bienestar program goals.
  • Ensure program is meeting state and federal standards of care and testing requirements
  • Complete GPRA's baseline, follow-up and discharge surveys within specified timeframes.
  • Maintain tracking tool for Tu Bienestar interventions and activities.
  • Assist with program reporting as needed.

Outreach :

  • Conduct in-reach and out-reach to identify and recruit clients for Outpatient services.
  • Work with Tu Bienestar Project Director to develop and implement outreach and community mapping strategy for reaching Latinas at risk for HIV.
  • Collaborate with other agencies, public organizations, and external personnel.
  • Coordinate 3-4 annual HIV-related events hosted at Casa.
  • Facilitate / Coordinate / Participate in street-outreach and other agency marketing strategies.
  • Represent the agency by providing education and participating in Boston community events.
  • General :

  • Maintain all client records and necessary reports.
  • Participate in weekly clinical meetings.
  • Bi-weekly supervision with Program Coordinator.
  • Assist in all reporting as required by Program Coordinator.
  • Other duties as required.
  • Job Requirements :

  • Must be available to work weekends and two evenings a week.
  • Ability to work both independently and as a member of a team.
  • Experience doing outreach to individuals at risk for HIV, addiction, mental illness, trauma and violence.
  • Crisis management skills.
  • CTR Certified or willingness to be certified within 3 months of hire.
  • Ability to provide HIV education, service planning, and health navigation.
  • Knowledge of care coordination and community resources.
  • Familiarity with substance use field including Motivational Counseling, Cognitive Behavioral Theory, AA / NA 12 Step Philosophy, and Stages of Change Theory.
  • Knowledge of co-occurring disorders.
  • Strong interpersonal and organizational skills.
  • Strong verbal and written communication skills.
  • Valid Massachusetts Drivers License required.
  • Computer skills, minimum Microsoft Word.
  • Experience in group facilitation
  • Bilingual (Spanish / English) a must.
  • Awareness of the intersection of culture, addiction, mental illness, trauma, health, and homelessness.
  • Ability to meet the physical and training requirements of the position.
  • Education / Training Required :

  • Bachelor's degree in social work, psychology, counseling or other Human Services related field preferred (or comparable experience).
  • Certified Alcohol and Drug Counselor / Certified Alcohol Counselor required or willing to obtain within one year of hire.
  • Certified phlebotomist, preferred.
  • CPR / First Aid Certified certified.
  • Salary Range : $36,000-$40,000
