Housing Case Manager (Multnomah County)

Cascade AIDS Project is excited to announce that we are hiring skilled and team-Oriented Housing Case Manager for Multnomah County. The individual works in CAP’s Housing and Support Services Programs and Multnomah County HIV Health Services Center to provide services that include home-based housing case management, goal planning, information and referral services, advocacy with and on behalf of clients, and eviction prevention for individuals and families living with HIV.

 

This is a Full-Time role working at our Davis St location and Multnomah County HIV Health Services Center in Portland, OR. The position will also travel regularly throughout the six-county service area for meetings and client home-visits. The schedule for this role is Monday-Friday 9am-5pm

Starting pay is $47,370.59. The Housing Case Manager’s Pay increases with tenure and the top of the pay scale is $55,170. This is a union represented position, so the compensation, benefits and conditions of work are collectively bargained.

 

Who You Are

  • Two years’ experience relevant to the position or bachelor’s degree in human/social services field (social work, public or community health, psychology) or related field  
  • At least two years of demonstrated success providing case management (or similar) services with knowledge of care coordination, client advocacy and community service navigation verified through previous employer references
  • Proven ability to maintain accurate documentation and ensure compliance with funder requirements
  • Previous experience working with individuals experiencing homelessness, mental health challenges, and/or substance use disorders
  • Full-time availability (40 hours per week), Monday through Friday, 9:00 AM to 5:00 PM, with limited flexibility (up to one hour before or after standard hours). This role requires four days in the office (split between CAP Davis St. and Multnomah County HIV Health Services Center with one day working remotely)
  • Demonstrated computer proficiency using Microsoft Office software (Word, Excel, Outlook) and working knowledge of the internet

 

What You’ll Do

  • Working with clients to develop housing plans.
  • Assisting clients in locating and securing affordable housing mediation with landlords.
  • Completing forms and entering data into the agency databases in a timely and accurate manner.
  • The Housing Case Manager works collaboratively and communicates effectively with clients, volunteers, CAP staff, and community partners. 
  • The Housing Case Manager will help Multnomah County HIV Health Services Center clients access housing services at CAP and work with those clients who are not eligible/not participating in CAP Housing programs access other housing resources in the community.
