Job Summary
Performs a variety of duties to provide administrative support to the Case Management Department.
Job Responsibilities
- Assists the Case Manager with facilitating discharges for patients on nursing units and identifies barriers to discharge.
- Coordinates post-acute care referrals and transfers and documents in the electronic medical record.
- Communicates with post-acute care facilities and other community agencies by phone and electronic medical record.
- Coordinates and schedules transportation and appointments.
- Completes required forms, regulatory letters, and other written communications and scans to electronic medical record.
- Maintains assigned calendar(s).
- Establishes and maintains filing systems.
- Orders and stocks supplies.
- Updates community resource information.
Qualifications
- High school diploma, General Equivalency Diploma (GED) or higher required upon hire
- No experience required
- No licensure/certification/registration required
