Description
- Screening, assessment, service planning, linkage, crisis intervention, de-escalation, managing relationship with a broad range of community providers, life skills teaching, transporting individuals to housing opportunities, assisting client in applying for housing, building connection with other agencies.
- Utilization of HMIS case management software.
- Ability to work independently.
- On-call coverage and evening hours required.
- Bi-lingual encouraged to apply.
- Bachelor’s degree from an accredited college, preferably in Social Work.
- Experience with conflict resolution and mediation.
- 2-years relevant work experience and demonstrated ability to provide effective services to clients required.
- Needs to be culturally competent across all populations. Bilingual a plus.
- Driver’s license with acceptable driving history.
