POSITION SUMMARY :
CCI Health Services is seeking a Client Support Facilitator for our Refugee Health Assessment (RHA) Program. The Client Support Facilitator will support refugee health screening patients by guiding them through the refugee health screening process and helping patients develop and adhere to care plans. The Client Support Facilitator will work closely with the RHA clinical and administrative teams, external partners, and program patients to provide information on social service availability and access for refugee health screening patients and encourage active engagement in their medical care. This role is essential in supporting CCI's commitment to providing high-quality, culturally competent healthcare services to our diverse patient population.
KEY FUNCTIONS & RESPONSIBILITIES :
- Support the clinic's refugee health screening process, including but not limited to coordinating required materials the patients should bring to their screening appointments, assisting with appointment registration paperwork, coordinating care referrals when needed, and scheduling follow-up appointments.
- Perform other clerical support functions, including but not limited to filing, scheduling, documentation of appointments or referrals, completing medical records release forms, and documenting no shows in the chart.
- Review the schedule the day before to better prepare needed registration updates to ask from the patient and any other paperwork as needed. Ensure provider schedules are distributed at the beginning of the sessions. Pull medical records and reviews prior to the patient's appointment, preferably by noon the day before.
- Collaborate with external caseworkers to verify patient eligibility, ensure patient preparedness for appointments, and identify and address patients' barriers to social services.
- Maintain strict patient confidentiality in accordance with CCI policies and procedures and HIPAA regulations.
- Assist with patient follow-up as necessary, including contacting patients with appointment reminders using interpretation in the patient's primary language.
- Collaborate with the healthcare team to identify and address any barriers to effective communication and care.
- Document messages that include a summary of the relevant details.
- Monitor status of reception areas, update appointment status by end of the day, and alert clinical staff of unusually long patient waiting times.
- Exhibit and promote a standard of excellence in the performance of all duties and interactions with all patients, visitors, clinicians, clinical employees, and external partners.
- Participate in coordination of care by assisting with behavioral health services, including obtaining initial authorizations required for Medicaid patients.
- Assist RHA Program Manager with quality reports and follow-up.
- Perform other job-related duties as assigned.
