Community Integration Specialist
Reports to: Director of Community Integration
Cedar Lake Residences (Oldham, Henry, and Jefferson County)
Full-time, 40 hours per week. Variable hours, may include evenings and weekends.

What you’ll do:
- Contribute to the development of individual’s Plan of Care (POC) with clients, significant individuals and case managers. Implement the POC with the objective of increasing self-reliance and independence. Encourage community participation and facilitates cultivation of friendships.
- Facilitate the ongoing implementation of evidence-based practices as it relates to industry standards.
- Facilitate the implementation of supported employment options, vocational rehabilitation services, and grow community access.
- Work collaboratively with individuals and family/guardians regarding community goals. Assess risk, safety or medical concerns that may be applicable. Encourage team members to provide supportive feedback related to the individual’s accomplishments and ambitions.
- Provide guidance to encourage a safe and sanitary living environment independently or with minimal supervision in regard to daily living skills, hygiene, housekeeping, cooking/meal planning, and personal assistance related to the individual’s POC. Facilitate involvement and discovery of interests, hobbies and building relationships with others in their community.
- Maintain production standard of 85% billable time as directed by Director.
- Facilitate transportation coordination/assistance to accomplish community goals, activities and scheduled appointments. Provide financial education and/or monitoring of personal finances surrounding budgeting responsibilities based on individual need.
- Encourage and assist individuals in exercising their rights while in community settings. Facilitate constructive ways to respond to conflict and develop strategies for resolution. Provide guidance and direction on social/judgement and health related issues that pertain to integration in the community.
- Maintain documentation which includes a full description of services rendered which entails a summary reflecting progress, outcomes, dates of services, implementation, goal projection and barriers/concerns with services. Help with preparation of paperwork needed for HUD and Medicaid within specified timeframes.
- Represent CLR in a professional manner providing ethical standards of practice for individuals we support.
- Perform other duties as assigned.
What sets you apart:
- High school diploma or equivalent
- Must be 18 years of age with valid Kentucky or Indiana Driver’s License
- Community Access certification preferred
