Clinical Admissions Specialist (Nursing Home Admission Liaison)

Cedarbrook Senior Care & Rehab is seeking a Full-time Clinical Admissions Specialist (Nursing Home Admission Liaison) for its Allentown Location.

Responsibilities :

• Evaluate Admissions Referrals for placement in the Facility with a focus on expediently converting appropriate referrals to admissions.
• Coordinate the Admissions process between referral source, resident, and resident representative / resident family while collaborating interdepartmentally to ensure Cedarbrook Admissions processes and procedures occur.
• Update relevant electronic systems with outcomes.
• May be required to occasionally travel to local referral source to evaluate a potential admission.
• Comply with relevant nursing home regulatory requirements.
• Other duties as assigned.
• Shift is 10 : 00 am - 6 : 00 pm Monday-Friday with flexibility for evening or weekend coverage as defined by Cedarbrook Administration.
