About the Role
At CenterWell Home Health, we are dedicated to providing personalized home care services to patients managing chronic conditions or recovering from injury, illness, surgery, or hospitalization. As a Home Health Aide, you will be part of a dynamic care team that includes nurses, physical therapists, occupational therapists, speech-language pathologists, home health aides, and medical social workers.
Your primary responsibility will be to provide direct patient care, including assisting with bathing, grooming, skin care, shaving, oral care, nail/foot care, and other activities. You will also communicate basic principles of nutrition, observe and record food and fluid intake when necessary, and report changes and document pertinent information and care rendered to ensure continuity of care. Additionally, you will practice acceptable infection control principles and willingly assist with other household duties as needed.

To be successful in this role, you should have at least one year of experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, home health/hospice agency. You should also be in good standing on the HHA Registry (if applicable) and have completed an HHA/CNA course to work for a Medicare-certified agency.
