Overview:
The HOPE Program coordinates behavioral health services for children ages 5-adulthood who are involved in at least two of the major service systems within Hamilton County. Those service systems are Developmental Disabilities, Job and Family Services Children's Services Division, Juvenile Court, and Mental Health. Staff are responsible for evaluating referrals, connecting clients to services, providing active care management and coordination, and discharge transition planning to ensure that clients continue receiving needed services.
This position is a full time position working 40 hours per week Monday - Friday.
Duties:
- Provide case coordination, care management, and case problem solving to clients
- Collaborate with all relevant stakeholders to ensure that referral is completed in a timely and seamless manner
- Provide care management services to referred clients
- Facilitate team meetings and provide leadership within the team
- Complete referrals to providers
- Monitor client needs and progress in treatment and continually reassess client to determine appropriate level of care and services
Minimum Qualifications:
- Bachelor's Degree in Social Work, Counseling or Mental Health Related Field
- Experience in working with chilldren with behavioral health concerns
Preferred Qualifications:
- Prior experience with child welfare, substance use, juvenile justice, and developmental disabilitites
- Licensure in mental health field
- Prior managed care experience
- Prior care management/care coordination experience
