HOW YOU’LL MAKE AN IMPACT
The Mental Health Practitioner Certified Peer Specialist provides peer support services to ACT recipients. The Certified Peer Specialist is an active member of an evidenced-based clinical team, providing medically necessary treatment, rehabilitation, and support services to identified recipients with severe and persistent mental illness (SPMI). This position provides a continuum of services including assessment, treatment, education, and support.
- Functions as a role model to peers; exhibiting competency in personal recovery and use of coping skills.
- Works with clients and other treatment staff to develop a treatment/recovery plan based on each client’s identified goals.
- Assists clients in developing empowerment skills and combating stigma through self-advocacy.
- Provides opportunities for people using mental health service to practice self-efficacy, through activities that emphasize the acquisition, development, and enhancement of skills needed to move forward in mental health recovery.
- Assists clients in articulation of personal goals for recovery.
- Utilizes tools such as the Wellness Recovery Action Plan (WRAP) to assist clients in creating their own individual wellness and recovery plans.
- Utilizes and teaches problem solving techniques with individuals and groups.
- Assists the client in obtaining decent and affordable housing of their choice in the most integrated, independent, and least intrusive environment.
- Assists clients in building social skills in the community that will enhance job acquisition and tenure.
- Responsible for care coordination, whether it involves coordination within CMMHCs programs or with another entity identified in the certified community behavioral health clinic (CCBHC) statutory language related to care coordination.
